If my grandmother has legal managing conservatorship rights to a child, can she sign that over to me?

No. Only a judge can give legal custody to another person.
However, since she has legal custody of this child she has the right to have some input regarding this child since she knows this child.
I recommend that you and the grandmother go to see a family law attorney in your county immediately.
Look on this website and on www.avvo.com.
If you have no money, you can look for a pro bono organization in your county. However, most are overwhelmed with requests for legal help and there is a long waiting list. You might have to wait 6 months to a year for help.
