guardianship of child

what forms do i need to have so that my 15 year old daughter can go live with my 19 year old daughter and my son-in-law?My 15 year old is uncontrolable,into drugs,sex.She hangs out with the wrong crowd,is abusive to younger sister.Skips classes.Her older sister is the only one she will listen to half the time.I only need something so the older can put her in school and get medical treatment if needed.Iam the custodial parent,we have'nt heard from there father in over three years.Have no idea where he is.Child support office can't find him either.

You do not want "guardianship".

You want the sister to become her conservator.

Dad would have to be notified. If you can't find him, then it would be through publication. Publication costs more and is a pain but it can be done.

Since you agree to let the sister have custody and the sister agrees to custody, it will be pretty easy. If Dad agrees to, it could be done very quickly -- there is no waiting period for a change of custody if EVERYONE agrees!
