How to legally change name

My husband and I found out that his mother is not his birth mother and there was no formal adoption done between his birth mother and the mother that raised him. We have since obtained his birth certificate which hadn't been able--name removed--locate for now obvious reasons. The name that is on his birth certificate is not the name that he has been using all his life. He can't for this reason obtain a passport or handle anything at the DPS office here in Houston, because the name on his ID and his birth certificate do not match. What form do we need--name removed--complete--name removed--legally have his name changed and how much will it cost. Thank you for your time.

You can find this information on the website for you local court; I believe it would be the district court. There should be sample forms and instructions.

I agree with Mr. Nance. Additionally, there are several attorneys on this website that practice in the Houston area. I would expect that they would quote a flat fee over the phone. There are costs and filing fess as well. Good luck.
