living arrangement

my daughter was adopted in Tx by my grandparents. I now live in Missouri and now she wants to live with me. The grandparents have agreed to let her live with me. What documentation do I need to have them sign for medical , education, religion and any other decisions that might have to made for my daughter

There is no "documentation" that can be signed for the care of a child. If you want to be the child's guardian, you will have to obtain an order of the court. Children are not puppies that can be passed around with AKC papers.
