Minor child

My 19 yr old single daughter is about to have a baby, what kind of form would she need to complete in order for someone to obtain custody of the baby in the unfortunate event something were to ever happen to the child's mother?

Your daughter needs a will to establish guardianship of her child in the event of her death. Since she is over 18, she is an adult, for this purpose. The biological father will have a stake in this as well.
