My mom is legally unstable to make decision because of an illness, what do i file and what is the process

You need to find someone who has standing to file for custody of you. If you have been living with your mom exclusively, then you can ask a grandparent, aunt, uncle, sibling, etc. to file a original suit affecting the parent child relationship under Texas Family Code 102.004.

You can also call CPS, but you might end up in foster care.
