What are my options at contesting/changing a court order/ruling where I signed under "agree as to form only" and NOT under "agree as to form and substance"?

You must give more information but generally it means that the court ruled against you so if possible, you must appeal.

It depends. If it was a temporary order, then you might be able to modify it. If it was a final order, then it can be appealed for 30 days. After that, it is a final order and not able to be appealed.
Because there are several important deadlines for a final order, you should see an attorney immediately. If you are even one day late in filing something, you may be too late.
