Parental Rights
How do I get the paper work and have it filed to have my daughter's father sign off all of his parental rights, have her name changed and yet have him still pay child support?

Not going to happen.
This is a serious and complex topic.
If his parental rights are terminated (not likely unless an adoptive parent is stepping forward to accept responsibility), then expect his obligation to pay child support also to be terminated.
