passport

i am the custodial mom.my 2 boys and i are planning to take a vacation( june 1st till end of month) outside usa .the father refused to sign the consent for renewal of their passport saying he wants the kids for 2 weeks before we leave then he`ll give consent.classes ends may 30th. i told him i cant because my vacation is granted on this month plus he moved to a new house and he refused to give me his new address and i dont have a direct contact with him because he changed his number and the new wife took his old phone.its in our decree that he provide me of his current number and address. he told my kids he dont want to talk to me again. our communication is thru the wife and i doubt she relays my message to him. he works in louisiana(they live in houston).our decree also state that he cant take the children out of hidalgo and cameron county.i always allow him to have the kids before when he visit.i dont want to violate our divorce decree because he might use it against me later. how do i get him to sign it .is there any other way i can do to get the passport without his consent.i read about getting a court order and i want to do it myself.what forms do i use and is the procedure long?your response will be highly appreciated
