what forms do i need to have my babys dad give his rights as a parent he is willing to sing any paper but we dont want any lawyer we want to do it ourselves

can you please advice me and tell me the name of the forms i need I have been doin researches and have several forms but dont want to miss any of the forms

I am confused. You don't want any help from a lawyer, and you want to do it yourself. Yet you realize that you need help, since you posted at this website. In your research, you might have learned that this is a process that is not quite so simple, and might require legal expertise.
