Power of attorney delegating parental authority.

Is a form that I have power of attorney delegating parental authority still a legal binding document if i have it signed in front of a notary with me and the parent there?

I am not sure what you mean by 'power of attorney delegating parental authority.' If you are referring to a document which permits medical providers to treat your child in your absence, then it is not required that it be notarized, but many hospitals have an internal policy which requires that it be notarized, to reduce their liability.
