I sell medical powers of attorneys and property power of attorneys. I cannot sign. Do you have someone (other than your husband) that can come and interpret for you.

The cost is $100 for medical power of attorney, directive to physican (if you want it), durable statutory power of attorney (to do business transactions).

I'm already booked all day Thursday and Friday. I have a few openings the afternoon on Tuesday or Wednesday afternoon.

I need:

full legal name

date of birth

address

phone number

email

Spouse's full name

date of birth

address

phone number

Does Medicaid have a special form they prefer? If so, I'm a notary and could just notarize it for you.

Take care!
