Releasing parental rights

My sons father wants to release his rights for my son so that my husband can adopt him. I need to know what exactly he needs to fill out? I do not know what it is called.

It takes more than a form to terminate a parent's rights. The father's rights are normally terminated at the same time the stepfather adopts the child. Adoptions are not something you can do by yourself. Please find an attorney.
