Looking for Samples of Legal forms
I wish to adopt my step-daughter, and we have contacted her birth father, who currently lives in Indiana and who is willing to relinquish his rights. However, I have searched online, and have not been able to locate a sample forms for the Affidavit of Relinquishment of Parental Rights nor for Termination of Parental Rights used in the State of Texas. Could you refer me to a site,and/or book that has these forms?

These forms should be available in your local law library.

You can go to the Harris County law library.
However, there is a reason I went to law school. This is NOT a simple legal matter.
Terminating a person's parental rights is one of the most difficult rights to terminate.
For example, once terminated a person is treated as DEAD toward that child.
I urge you to retain an attorney to assist you through this process.
