Sharing Custody

I am the primary conservator of my 15 year old son..

He has decided to live with his dad and i need to know how to make a ammendment in my case for us to share custody 50-50.... Is there a do it yourself packet that i can get as long as we agree on this??

I don't know of the existence of a do-it-yourself packet on this subject. You can find forms books in the law library if you want to do it yourself.
