Should I sign the waiver if my name and my TX ID is wrong or just cross it out and fix it?

You're better off filing an ANSWER. It accomplishes the same purpose as the waiver (i.e., you enter an appearance), but you'll be entitled to notification from the court whenever something happens (for instance, if a hearing is set).
