Legal Question in Family Law in Texas
My sister is in hospice care and we are trying to figure out who has the highest level of legal rights to handle her care.

You have not provided enough info to answer.
If she is mentally competent, she will handle her own affairs.
If she's married, it's her spouse.
If she is in a coma and single, then it gets complicated.
Someone might need to be appointed to make decisions for her.
Of course, the hospice has to agree to follow the decisions -- you might to deal with their legal counsel -- they often worry about being sued.
You need to meet in person with an estate planning attorney in your county to discuss the situation.
