My son is fifteen and wants to come to live with me. What forms do I need to get custody of him?

There aren't any "forms." You would file a petition to modify conservatorship and ask the Court to find that it's in the child's best interest for you to have the right to determine his residence. File the petition, get Mom served, and see what happens.
