Legal Question in Family Law in Texas

My son has moved in with me and I am looking for a form that he can sign stating that he wants to live with me. He is 16 and of age to make his own choice. Do you know of such a form

There is no such form. A child (someone under the age of 18) never gets to decide where he wants to live. At 12 he can sate a preference, but the decision is always up to the Judge.
