can my ex-wife still use my last name as her own if we have been divorced for 12 years and she has been married twice since then?

Yes. In her subsequent divorces, she had the right to ask the judge to legally revert to a prior name. Likewise, in her subsequent marriages, she had the right to retain the name she had been using.
