Waiver of service for a name change

I submitted an original petition for the name change of a minor today to the County of Rockwall District Clerks office. Included in the papers were a consent to name change form signed in the presence of a notary by my son's father. (I used an online paralegal prepatory service to create the docs.) The judge told the clerk that I need to timely submit a ''waiver'' and an ''agreed order'', which I am assuming from my research today is a ''waiver of service'' (which doesn't make sense since the father already signed the consent form/notarized) and an ''agreed order granting petition for the name change of a minor''. Where can I get these forms on line or a sample?? All of the lawyers I contacted today are expensive and charge even for a consultation. I cannot afford this. I have scoured the net looking for samples but cannot find any. Thank you for your help.

I am a lawyer also and I charge for my legal services -- just like most lawyers or doctors or hairdressers, etc. But when I need forms, I go to the local law library if my online paralegal preparatory service doesn't have them. Sounds like the illegal practice of law to me.
