Steps to file under FMLA

Once the FMLA paperwork is filled out by my doctor and

others, how do I submit it? Address? Or do I turn it into

my employer?

You need to turn it intoyour employer and then make sure that you provide periodic stauts updates and that the doctor note contains a diagnosis and a return to work date.
