There really is no specific document that tells people what to do with your body. The best thing to do is to make your wishes known to others before your die. Set them down in writing. If this is important to you, and you are rich, you can include a provision in your will that all gifts are made on condition of your body being disposed of in manner X, and that if your body is not disposed of in manner X, then all gifts in you will go to ....... Then make sure that everybody who might take under you will knows about this provision. If you are not rich, just make sure everyone knows hope they will honor your wishes.
You might make things easier for them by making the arrangements in advance and again making sure that people know about them.
There is a document called a durable power of attorney for health care that designates a person to make health care decisions for you. Under the law, the hospital must put you on machines and run up your bill unless you sign an advance directive.
