Legal Question in Civil Litigation in Texas

power of attorney

I am located in Texas and have a question on the validity of a Power of Attorney. My question is, Does a Power of Attorney have to be witnessed, notarized, both or neither to be a valid document? and Does it have to be filed anywhere?
