Legal Question in Civil Litigation in Texas

I need to submit a Bill of Discovery for information in order to file a small claims complaint. Clerk tells me to submit it in writting. Is there a specific format, structure, or document template to use for this petition? Thanks

Put the style of the case on top (see the petition: cause number xxxxxx, Jane Doe v. Joe Blow, Justice of the Peace Court number XXXXX) then put the title of the document - DISCOVERY - then put the body. Put a signature block with your name and contact information and a place for you to sign. Follow with a Notice of Service to whom and the manner of service, and send a copy to the other party.
