sue the insurance company or the lady

I was driving in parking lot and stopped while seeing her car started backing up from her parking spot. When I saw her car was backing up too fast toward my car, I horned three time but could not make her stop. The left corner of my bumper was demaged.

She told her insurance company a different story. My claims was denyed even though the police report said she was backup

without safety.

If I will go to small claims court, whom should I sue? The insurance or the lady? Which court should I select, where the accident happens or where she lives?

You sue the lady. Send a courtesy copy of the filed Complaint to the claims adjuster.

Small Claims must be filed in the county and precinct where the defendant lives. Make sure you give proper notice of intent to sue before you file your complaint. Your JP should have information for you about filing in Small Claims.
