statue of limitations
if someone forges my name and uses my ssn # to take out a life insurance policy on me without my knowledge and I find out about it later how much time do I have to file charges.

I have no idea. Instead of worrying about the statute of limitations call the police and file a report and notify the insurance company in writing. There is only one reason to get a life insurance policy and that is to collect on that person's death. If you are in good health then you should draw your own conclusions as to that person's motives. Call the police immediately.
