What happens if a tenant has to break a lease and can't afford to pay out the rent?

The landlord will make demand for payment of the balance of the lease, and if you don't pay it, they have the right to sue you. You can try to negotiate the amount owed and work out a payment plan. If you can't then they have to figure out a way to collect the money from you, that you don't have.
