What form do I use?

I wrote a book and would like to include real names. The person has read the book and given verbal approval. What form do I use for him to sign?

You will need a form that waives claims against you and memorializes their consent to your use of their name in the book and confirms that they have read the final version of the book. I don't know if LawGuru has this type of form. If not, an attorney can prepare it for you.
