My ex-boyfriend owed me a hefty amount of money. I never kept receipts because everything was paid for with cash; but he, his grandmother, and I all knew that he owed me just a little under $1000. I just recently asked his grandmother to pay me back $800 and without any dispute she sent me a money order of that amount.
Later on, some other personal problems arose between my ex and me and in turn now his grandmother is considering suing me for extortion of the money just because he is upset with me. They told me that they will falsely claim I threatened his grandmother that I would destroy my ex's family if I didn't get the money, but that is completely untrue.
Will they really have any grounds to sue me on?

You need to talk to a lawyer.
I learned this in law school -- Anyone can sue you at any time for any reason.
If you are sued, you will have to defend yourself in court. Just because you are sued, it does not mean that they will win. A judge will decide if they have a valid case.
However, I believe that they are just threatening you.
They gave you the money. Now they are unhappy with you.
