what form would I use to add my husband to the title of my home which is paid for

In general, a quitclaim deed can transfer any interest one has in property to another ("I am giving any interest that I have in Land ABC to person B"). A general warranty deed ("I am giving A and I promise that the title is good to B").
Between Husband and wife, a quitclaim is sufficient to give him what you have, subject to other creditors or claimants that you may not be aware of (mechanic's liens, tax issues, etc.).
Additionally, be careful to make sure that the land is either transferred, shared, or divided as you want it to be.
