Lien on Second Mortgage

We sold our home in Texas. The man that bought the home signed a Real Estate Second Lien Note with us agreeing to pay ''x'' $ per month. He has not made payment and I want to put a lien on the home in the county the home is located and they said I have to have a form completed and then file it.Is the note he signed what I file or is there another form? Where can I get a sample form to complete. Is there any other way to collect this note. I have been told he may let the house go back and I don't think they will get what he paid for it, so I'd be out my money if the mortgage company does not get all of their money right? I am really upset and don't have the money to hire an attorney to do this, can I do it myself? Is there anyone that might give me a form to use? Thank you for your assistance
