If there are three lienholders on the same deed of trust, do all three have to sign the release of lien?

Almost certainly. Depending on the language in the DoT, a court might or might not hold that all the named beneficiaries must sign -- but the important point is that no title company is going to issue a policy to a subsequent purchaser without all three signatures on the release.
