There are a number of ways to do this. You can pay off the mortgage, or refinance it in your own name. She can deed it to you subject to the mortgage (provided the bank gives written permission to do so.) Or, she can leave it to you via a will or a ladybird deed.

I'm assuming she's not on Medicaid.
