Process to convey half of home ownership

My husband purchased our home in 1998. We married in 2001. We refinanced the home in both names in 2003. What kind of deed do we need to file to transfer half of the home ownership to me? Is there a place where I can find a form online? Thanks!

A lawyer needs to prepare the deed. Should cost you no more than $100, if you can provide him the needed information in one communication. You also need to contact the lender and get its approval, in writing, BEFORE doing this, as any transfer of ownership likely violates your mortgage.
