Special Warranty Deed

Is this the form I need to take my husband off the deed to the house.

Your question does not show me the form. Do not use a Quit Claim Deed. Have him sign a General Warranty Deed conveying all of his interest to you. A title company or lawyer should charge around $100 for the document and filing fee should be around $20-24.
