Application requesting approval of executrix fees

Please advise how I should sign an application requesting approval of executrix fees. I have the No. of the estate at top of page. In the Estate of (mom's name) on the left, and in the Probate Court of county, Texas to the right.

Then I put I, (my name), request approval for $500 for the executrix fees for the 2003 volkswagon, and 2002 toyota. I herby request your approval.

I don't know how to end it? Do I sign my name or do I put a line for the judge to sign or do I do both? Please advise.

The person requesting the relief signs the application (or attorney), and the judge signs an order. If you aren't familiar with how to prepare the documents, I suggest that you hire an attorney.
