How to change last name in Texas

I am already divorced, but the name change was not included in the decree. How can I now change my name back to my maiden name? I saw the change of name forms on your site, but there were PETITION and ORDER of adult name change. I didn't know the difference. If I order these is this all I need? Please help. Thanks,

You return to the court which granted the divorce with a request to restore your former name.

One makes a request of a court by filing a Petition stating the relief requested.

A hearing [not always] is held or a determination made concerning the request, by a judge, and usually it is granted.

A court grants a request by filing an Order stating what the decision of the Judge is.

You can probably do this yourself if you have some advice from your local public defender or a referral from your local bar association.
