EIN trust instructions

I have read the IRS instructions on filling out the form & requesting an EIN to establish a trust. The trust will be established for my mother. I am the trustee.

1-The instructions state to put the legal name of entity (or individual) for whom the EIN is being requested. Is that my name or my mothers?

2-The instructions state that under the blank for TRUSTS ''Enter the name of the trust as it appears on the trust document''. I assume that is the will but the will only states that a trust will be established for my mother's future wellfare, etc.... What would the name of the trust be?

1. Would be the name of the Trust.

2. The Will is the trust document.

3. Typically you would name it The Jane Doe Testamentary Trust.
