can you have an executor of an estate and a seperate person with the medical power of attorney?

I'm not sure what you're asking. You can appoint an executor in a will that you are drafting. You can also appoint a medical power of attorney in a separate document. The proposed executor and the person acting under medical power of attorney don't have to be the same person.

For example, Albert drafts a will. Albert can appoint Bob as an executor in that will. Albert can also appoint Charlie to act on Albert's behalf regarding medical decisions, by executing a medical power of attorney.
