Can a family member insert themselves as an executor of an existing will after the will's owner is diagnosed with alzheimer's?

Absolutely not! Only the person who made the will can alter it. Changing someone else's will as you have described is a criminal offense. After there testator dies, if there is no executor available, it is a simple matter to petition the the probate court to appoint an executor. If no one objects, the court will likely appoint the family member who volunteers.
