Guardianship for Daughter with Down Syndrome

My daughter has down syndrome and she will be 18yrs on 12-08. I was informed that my wife and I need to set up a guardianship for her soon. I don't know what all is involved but if you could shed some light, I would appreciate it. Is there forms I can fill out myself without an attorney?

Mr. Leon is correct that it is a fairly complicated procedure and one that needs to be started very soon. Unless there is a guardianship in place, once your daughter turns 18 you no longer have right to make legal or medical decisions for her. THis is a fairly complicated and ongoing procedure. I would not recommend your doing this by yourself. You should contact either your local bar association or a probate lawyer in your area to set this up.
