How can I give legal authority to my significant other to make medical decisions for me if I am unable to make them for myself?

A medical power of attorney and a directive to physicians. I advise you to see an attorney for these documents. If you are wise enought to be considering these choices, you must be very interested in your decisions being realized. If you do it yourself with internet forms, are you *sure* it will hold up in case of dispute. Guess who will suffer it it's not done right.

You need a medical power of attorney and physician's directive (living will). Although you can download forms to do these, most attorneys will do them for you for a very reasonable cost, or as part of an estate planning package.
