Medical Power of Attorney

My mothers best friend lives here in San Antonio. Her children live in other states. She would like my mother and myself to have medical power of attorney in case she has to go into the hospital and can not make dessions on her own. She picked up the forms at a local office supply store. I would like to know if this document has to be noterized. Her children are in complete agreement with the idea and infact were the ones who picked up the forms.

Obtaining a valid Medical Power of Attorney through an attorney who knows what he or she is doing is usually not that expensive. Forms, particularly ones picked by persons from out of state, are probably worth about the cost of the paper they are printed on.
