Legal Question in Wills and Trusts in Texas

I've seen a few of these cases in my office. Unforetunatley, I can't answer this without knowing more:

1. Who was the beneficiary listed under the policy?

2. When was the policy purchased (before or after the marriage)?

3. Is there a divorce decree or other court order concerning the policy?

4. Was this policy a work benefit or ERISA based plan?
