transfer deed to trust

i have just paid off my home mortgage. what form do i use to transfer my home deed to include it in my living trust - a revocable trust with my name. i understand that i may do this by filling out some kind of transfer form. thank you for your help. can you include any special instructions i may to accomplish this.

You'll need a special warranty deed to transfer the property from you to yourself as trustee on behalf of the trust. Use a real estate lawyer to assist, since you don't want to risk screwing up the chain of title to real estate.
