On a will, is the name of the attorney on the cover page, the attorney that drew up the will? What happens to the documents and paperwork if an attorney becomes disbarred?

Usually, the name on the cover page is the office in which the will was prepared. There's no rule on that, however. If an attorney later gets disbarred, the will is not effected. If it was a valid will, it remains a valid will. Prudence would suggest that you have the will reviewed, just to be safe.
