In order to have a self-proving will (admissible in Court), you must have 2 witnesses (not beneficiaries of the will) and signed before a notary by the witnesses and the person making the will (the testator). You would be better off to get this done by an attorney; otherwise, you run the risk that the will be done incorrectly and not effective as a will.
