I am not sure if there are any such forms, other than obtaining a temporary restraining order through the Courts, however, you will have to show specific acts or concerns. You could certainly prevent the 18 year old from coming onto your property and if he does, then the police can be called for his trespassing. As the parent, you certainly have the right to tell you 15 year old to not have contact with the 18 year old, however, you will have to decide how effective that might be. If there is any type of sexual contact between them, then the 18 year old may have committed a crime and may be arrested. I would probably make sure that the 18 year old knows that you have disapproved of the relationship and will be watching for any contact, while also letting your 15 year old know that also. If you think that things are continuing and potentially becoming sexual, I would contact the police and let them speak with the 18 year old and investigate their actions.
