Setting up a Foundation in Utah

We are seeking to set up a non-profit foundation in Utah to be called ''The Foundation for the Advancement of Humanity'' which will seek to advance individuals served in both areas of human need- both in the physical as well as in the spiritual spheres.

I need information on what type of forms need to be filed and with whom (state and federal) to set up this non-profit foundation and what type of administrative setup (board, trustees, etc.) that will be required by the state of Utah and/or the federal governement. I also need any advice on how to set up the financial part of the foundation to meet the applicable state and federal requirements.

You will probably need to incorporate as a not for profit corporation with the State of Utah - a foundation does not need to be incorporated, but will need organizing documents and other materials to create a 'charter' for non-profit status. You will also need to file and register with the IRS requesting non-profit status under the terms of your charter and the IRS for the correct classification. Contact the State and the IRS for the proper forms.
